Step 1: Open the visit and find the Visit Assets panel
From your Visits page, click into the visit for the patient whose chart you want to generate.
On the right side of the visit page, you'll see the Visit Assets panel with a Generate Documents section at the top.
Step 2: Choose your dental template
Click the Select documents… dropdown. Two HappyChart templates are available:
HappyChart – Canine Dental
HappyChart – Feline Dental
Pick the template that matches the patient's species.

Step 3: Confirm which clips to reference
Below the template selector, the Library lists every dictation clip and upload attached to the visit. Make sure the box is checked next to each clip you want HappyChart to use as the source for the chart.
Tip: Only check the clips that contain the dental exam. Including unrelated clips (like a separate wellness exam from the same visit) can introduce noise into the chart.
Step 4: Click Generate
Once your template and clips are selected, click the green Generate button.

HappyChart will process the audio and produce your chart in a few seconds.
Reading your generated chart
The chart is laid out anatomically: Right Upper and Left Upper quadrants on top, Right Lower and Left Lower on the bottom, with a tooth diagram down the middle. Teeth are numbered using the modified Triadan system (101–411) and grouped by Incisors, Canine, Premolars, and Molars.

What the colors mean
Each tooth row shows either NSF (No Significant Findings) or a specific finding pulled from your dictation. Findings are color-coded so abnormalities stand out at a glance:
No highlight, "NSF" — Normal, no findings
Yellow — Fractured Tooth
Pink/red — Extracted Tooth
Teal — Treated or notable finding (e.g. scaled and polished, doxirobe applied, focal gingivitis)
Light Grey/dotted — Absent/Missing Tooth
Additional findings text fields
Each quadrant has an open text field below it labeled "[Quadrant] additional findings". Use these for any free-form notes that don't tie to a specific tooth — overall plaque distribution, soft tissue observations, occlusion notes, etc.
Switching between charts
If you generated both a Feline and a Canine chart, you'll see them as tabs above the chart title. Click any tab to switch.

Scroll below the tooth diagram and you'll find two auto-generated sections:

Pulled directly from your dictation:
Occlusion — Normal, malocclusion class, etc.
Periodontal disease stage (0–4) — Overall stage with a short narrative
Gingivitis Index (GI) — 0–3
Calculus Index (CI) — 0–3
Oral findings — Synthesized clinical impression
Radiographic findings — What was seen on rads
Procedure Report / Plan
Procedure Report — Full narrative of what was done (anesthesia, blocks, extractions, scaling, suture material, etc.)
Prescribed Medications — Listed as line items
Recheck / Additional Notes — Follow-up instructions
Every field in both sections is directly editable — click into any line and type to edit. Hover over a section to use the copy icon on the right to copy that block to your clipboard.
Printing or saving as a PDF for the client record
Click the Print icon in the toolbar to open the print preview. You'll see the chart laid out on letter-sized pages with the HappyDoc logo, patient name, date, and doctor in the header — ready to hand to the client or save for the medical record.

Save as PDF to your computer
Print to a connected printer
Open PDF in Preview for a quick look before saving
Common questions
Can I edit the chart after it's generated? Yes. Every tooth row, every additional findings field, and every line in the Summary and Procedure sections is editable. Just click and type.
What if HappyChart misses a finding or gets one wrong? Edit the affected tooth row directly, or click Regenerate if you've updated your dictation clip. Use the 👎 rating button to flag persistent issues so we can improve the model.
Can I add a new clip after generating? Yes — click Add Asset next to "Included Assets" at the top of the chart, attach the new clip, then click Regenerate.
Why are some teeth missing from the diagram? The Feline template only includes teeth that exist in the feline dental formula. If you're charting a dog, switch to the Canine template. If a real tooth is absent on the patient, mark it as Missing in the appropriate row.
The chart isn't generating. What should I check?
Confirm at least one clip is checked under Library
Confirm the visit has audio that includes the dental exam
Confirm the patient's species matches the template you picked
